How to Enroll a New Client with Modus (1)
If you are requesting a ride for a first time Modus rider, we have an enrollment process that must be completed prior to filling out the actual Ride Request, which requires that you obtain the client’s consent. Since you will be requesting rides on the client’s behalf, you will need to obtain their consent.
Reminders
- Modus must receive these forms no later than 5:00 PM, the weekday before the ride is needed. These forms only need to be filled out one time.
- The provider is responsible for explaining each document with the client and submitting the completed forms to Modus using one of the three options listed here.
- Once Modus receives the client’s consent, Modus sends a text message to the rider notifying them that they have been enrolled and requires confirmation that the client received the text message.

Obtaining Client Consent (3)
The Provider is responsible for obtaining the client’s consent and all Enrollment Forms must be consented to and submitted to Modus by no later than 5:00 PM, the weekday, before the ride is needed. These forms only need to be submitted one time, which is at the time of the enrollment.
Providers may obtain client consent three different ways: Digital Consent, Paper Consent, and Provider Attestation.
