Provider Attestation (Client or Client's Guardian Consented Verbally)

If the client does not have access to the internet via a smart phone or email and cannot visit you in person, you can use consent by attestation. Follow the scripts attached in the ‘Provider Attestation’ Ticket to obtain a client’s consent to authorize each form on the client’s behalf. Then, attest within Modus that you read the script, allowed the client to ask questions, and verbally received the client’s consent to sign on their behalf. This ticket must be submitted prior to signing and submitting any forms on a client's behalf.

If completing the Provider Attestation, the Provider Attestation ticket must be submitted with the enrollment ticket. The provider must still submit forms they signed on their client's behalf. These forms can be signed and attached to the enrollment OR they may be submitted via DocuSign.

  • If the provider is consenting on behalf of a client under the age of 18, then the provider must complete the 'Provider Attestation (Legal Guardian Consent)' Help Topic.
