Paper Consent (Client Signs the Form and Provider Uploads Scanned Form to Modus)

If the client signed a physical copy of the Enrollment Forms (download available on this page), you can scan the document to your computer and upload it into the ‘New Client Enrollment Ticket’

  • Modus must receive these forms no later than 5:00 PM, the weekday before the ride is needed. These forms only need to be filled out one time.
  • The provider is responsible for explaining each document with the client and uploading the completed forms in a New Client Enrollment ticket.
