How to Enroll a New Client with Modus

If you are requesting a ride for a first time Modus rider, we have an enrollment process that must be completed prior to filling out the actual Ride Request, which requires that you obtain the client’s consent. Since you will be requesting rides on the client’s behalf, you will need to obtain their consent.

Once Modus receives the client’s consent, Modus sends a text message to the rider notifying them that they have been enrolled and requires confirmation that the client received the text message.

Obtaining consent

The Provider is responsible for obtaining the client’s consent and all Enrollment Forms must be consented to and submitted to Modus by no later than 5:00 PM, the weekday, before the ride is needed. These forms only need to be submitted one time, which is at the time of the enrollment.

Consent options are categorized into three sections: Digital Consent, Paper Consent, and Provider Attestation.

Steps to complete enrollment

  1. The provider submits a New Client Enrollment ticket.
  2. The provider explains each document with the client and uploads the completed forms in a New Client Enrollment ticket. Any forms signed via DocuSign do not need to be added to the New Client Enrollment ticket.
  3. The provider submits the New Client Enrollment ticket. If submitted successfully, the provider will automatically receive an email confirmation.
  4. Once a client is added to the Modus database, the provider will receive another confirmation that their client has been enrolled with Modus.
  5. FOR CLIENTS - Upon enrollment, Modus sends a text message to the rider notifying them that they have been enrolled and requires confirmation that the client received the text message.

Reminders

  • Modus must receive these forms no later than 5:00 PM, the weekday before the ride is needed. These forms only need to be filled out one time.
