XX. How do I schedule a ride for an existing Modus client?

Once the client is enrolled in Modus, the Provider (i.e. counselor, case manager, etc.) can request a ride by submitting a Modus Ride Request ticket online.

  • This ticket must be filled out prior to every Modus ride unless the appointment is reoccurring and the original ticket clearly indicates such.
  • All ride requests must be made by no later than 5:00 PM, the weekday before the ride is needed.
  • Once the request has been processed, you will receive a confirmation email with the details of the trip. Please review the details and alert Modus if any changes need to be made.
  • Prior to the scheduled Modus ride, Modus will send the rider a text message to confirm their ride. This message also includes details of the trip, such as date and time of pick-up. 

*Note: Request forms may have more details added due to Modus' COVID protocols. The information required, however, is still generally the same.
