XXIX. My client was enrolled with Modus prior to June 1, 2020. What forms are required before my client can resume services?

If a client was enrolled with Modus prior to June 1st, 2020, they are required to sign and return the following forms: 

  • Rider Agreement During COVID-19
    • To sign the form electronically, click here
  • COVID-19 Acknowledgement and Liability Waiver. 
    • To sign the form electronically, click here

An email is required to fill out the forms electronically. If your client doesn't have an email, they are welcome to input our email for clients which is ride@modustulsa.org.
