VIIII. How far in advance do I need to request a ride for my client?

All Ride Requests must be submitted to Modus by no later than 5:00 PM, the weekday before the ride is to occur. Furthermore, Modus is requiring all clients new clients and any clients who were enrolled prior to June 1, 2020, to sign our new Rider Agreement and COVID-19 Acknowledgement and Liability Waiver. All forms must be signed and returned to Modus prior to requesting a ride for your client. The new forms can be found here and can be signed electronically.
