Digital Consent (Send Clients a Link via Email or Text)

If the client is not with you and you do not have the signed forms to upload, you may request consent from the client via Email or text message (SMS). Below are the links to the enrollment forms; this is where they provide their electronic signature via DocuSign.

If your client is signing the forms electronically through DocuSign, then you can send the links below directly to the client via text or email. For tips on getting electronic signatures, click here

Tips for Getting Electronic Signatures

There has been some confusion on how to utilize DocuSign to gain the proper signatures for your clients. Some helpful tips for gaining these signatures are below.

  • Emailing the links
    • Sending the DocuSign links directly to your client by copying (ctrl+c) and pasting (ctrl+v) the links via email.
  • Texting the links
    • Sending the DocuSign links directly to your client by copying and pasting links via text message.
    • For text message - if you click on the link with your finger and hold for a few seconds, your phone should give you the option to copy the link; to paste you'll press your finger in the message box and hold for a few seconds and it'll give you the option to paste the link.
  • DocuSign requires an email address to complete the forms electronically.
    • If your client doesn't have an email, they are welcome to input our email for clients which is ride@modustulsa.org.
